What facilities usually ask for
Requirements vary by facility and state, so ask each one for its list. Having these ready makes applying faster:
Most applications
- Photo ID and Social Security number
- Medicare, Medicaid and any other insurance cards
- Names and phone numbers of doctors
- A current list of medications and allergies
- Power of attorney or health care proxy papers, if there are any
- An emergency contact
Often also needed
- Nursing homes: a doctor's order or hospital discharge papers. Nursing homes that take Medicaid must also complete a federal pre-admission screening (called PASRR) before admission.
- Assisted living and memory care: a recent physician's assessment and, in many states, a tuberculosis (TB) test.
- Paying with Medicaid: proof of income and assets. Medicaid approval can take weeks, so start early. See help paying for care.
Only give personal, medical or financial documents to the facility itself or to a government agency. Never pay anyone who offers to “get you in.”
